Remote Learning Assurances Forms

Remote Learning Assurances 

The State Board of Education has decided to have parents sign an assurance note rather than logging the daily minutes. The assurance note means that anytime a student is working remotely (quarantine, hybrid learning, remote learning) that you will support the learning process. We need to have a form completed for each student. 

 

Directions for Hybrid Students and Families: 

Please list your student’s name on the form and sign the bottom. Complete one form for every child you have at the high school. Each student must have their own form. Track B students need to return the form on September 25th. Track A students need to return the form on September 29th.


Directions for Fully Remote Students & Families: 

Please list your student’s name on the form and sign the bottom. Complete one form for every child you have at the high school. Each student must have their own form. Use the stamped envelope to return the form to school.  Please mail by September 30th.  Forms are due October 2nd. 

 

There are extra paper copies of the assurances form in the office. If students are unable to return the form in person, there are two options:

1) Have a parent/guardian sign the paper copy and take a picture of it. Send the picture to your Advisory teacher or hs.admin@usd470.com.

2) Use an extension like Kami to have your parent/guardian sign it electronically.  Send the form to your Advisory teacher or hs.admin@usd470.com.
